Excerpt
The planning and operation of the Epidemiologic Followup Study to the first National Health and Nutrition Examination Survey (NHEFS) could not have been accomplished without the valuable assistance from many individuals and groups. Space constraints make it impossible to recognize all who participated in the planning, development, and conduct of the varied aspects of the survey; nonetheless, the authors are grateful for the support received, and apologize to those omitted.
