Few people now doubt the magnitude of the suffering caused by tuberculosis - in this decade alone tuberculosis will probably be responsible for some 30 million deaths. It will cause more disability and mortality among adults than any other single infectious disease. However, we live in a world where tuberculosis programmes are at best understaffed and underfinanced; where the diagnosis of tuberculosis - relying as it does on 19th ccntury technology - is insensitive; where treatment completion rates are often well below SO%; and whcrc thc means of ensuring adherence to therapy remain unclear. Consistently, the world community has failed even to implement those effective and well tried measures against tuberculosis that havc existed for almost 40 ycars
