HIV fuels the TB epidemic, since HIV promotes progression to active TB in people with Mycobacterium tuberculosis infection, either acquired recently or in the past. The implication for TB control in high HIV prevalence populations is that prevention of HIV is crucial to control TB. Since TB is a leading cause of morbidity and mortality in people infected with HIV, the implication for HIV/AIDS programmes is that TB is a substantial part of HIV/AIDS care. However, so far TB and HIV programmes have largely pursued separate courses. It is necessary to improve the joint support of TB and HIV programmes for general health service providers, in responding effectively to the needs of people infected with HIV. Participants at the “Global DOTS Expansion Meeting” held in Cairo in November 2000, called for the establishment of a Global Working Group on TB/HIV. The goal of the Working Group is to reduce the burden of TB in high HIV prevalence populations. The Working Group, coordinated by WHO, is one of the six working groups established under the auspices of the Global Stop TB Partnership. This first meeting of the Working Group is the first step in harnessing and coordinating global efforts to provide a more effective response to the problem of TB in high HIV prevalence populations (TB/HIV).
