Preface: This report presents a detailed description of a major portion of the research undertaken to redesign the National Health Interview Survey (NHIS) for the period 1995–2004. The NHIS is one of the major data collection programs of the National Center for Health Statistics (NCHS). Through NHIS, information concerning the health of the U.S. civilian noninstitutionalized population is collected in household interviews throughout the United States. NHIS has been in continuous operation since 1957, and its sample design has been reevaluated and modified following each of the last four decennial censuses of the U.S. population.
